I don't know how to do it.if you just use the front part, what do you do for leverage as you would have with a peg to lift up on the shifter?

Leverage is not a problem. Its a natural motion to use the toe portion only. I've had mine off for several years, gives me more room for my big feet.
 
I took mine off the first week; same reason.
In that case, what do you do with your foot when you're pushing down for 1st or pull up for the rest? With a peg, your instep sits on it and the front of your foot just pivots up or down.

I just pivot off my heel, size twelve boot, it works fine.

I had the heel shifter on my storm and much of the time didn't use it call it brain freeze i'd be shifting and forget what to do so i would just shift normal and the upshift felt normal.

So you plant your heel on the board and lift or stomp the shifter with your toe?
