Anyone Else Lost a Heat Shield Yet?

Got back from a ride and this is what I see. I remember something hitting my boot and wondering, at the time, what it was.

About $60 + tax + shipping for one shield, two grommets, and two bolts.

So the grommets (#5) go in the hole in the exhaust and have the threads for the bolts. My top one was loose, so I spun it around and kinda popped it into the hole and then torqued everything up- before the last ride. I think once the exhaust gets hot the hole on the pipe expanded and no longer held the grommet in place.
I don't want to mess with the other two, which are fine, to better understand, so I'll have a look when the new parts come. I need a better plan to keep the grommets from detaching. Would a bit of JB Weld work- yah think?

No, but I know what I’m torquing the next time I’m in the garage.
the issue is cheap nuts and weld on it, it tends to slip the thread/rip the thread. of nut, though torquing will help but dont rely on it as permanent solution.
 
I rode back the next day and found it- middle of the road and had been run over. I don't see how the grommets are held in place by the holes in the exhaust (well, they were not)....

It would make more sense if the grommets were turned around and came in from the back of those holes. I have a feeling that is the way they are supposed to go and some numb nuts reversed them in production. I'll see if I can stick a mirror behind the others to see how they were installed. Curious if anyone else knows the setup on their bike.

A few of mine got loose and I had a buddy spot weld all of them. No problems after that. 🤬 It would make way more sense to have the flange underneath.
