Uneven Brake Pad Wear

I recently had to have a rear tire change-out while on the road and noticed something odd while the tire was off. My rear brake pads are wearing at an angle. By that I mean that the face of the pad is not parallel to the backing plate. It is thicker on one end than the other. Anyone else had this problem? If so, what was the cause and what did you do about it?

I suspect that a thorough cleaning and a little lube will sort it out as it just returned from a 4,400 mile Hanso / RAA road trip and probably has a fair amount of crud around the rear caliper which is not allowing it to float properly.
 
@R3Tex , In the past I've found that the pads are not sliding on the pins or ledge and the pistons are pushing the bottom of the pad causeing a wedge shape wear on the friction material.
