Circlip on rear hub

I was changing my tires on my 09 Touring last night and when I pulled the rear wheel off the bike the Circlip fell on the ground. It must have came out of it's groove. Doesn't look like any damage has occurred. I bought and installed a new clip. I am assuming it was never put in correctly from the factory. Bike only has 6k on it.
Has anyone experienced this problem?

It secures the Driven Flange assembly to the rim.

My 2005 Honda ST1300 had the same set up on the rear wheel. The clip is strong and fits in the groove very tightly. There are only three ways the clip can come out. Improper installation, clip breakage or you remove it. My bet is it was never installed right at the factory. Not good!
